Samenvatting

Hoewel de intubatie geen dagelijkse handeling is voor de kinderarts, kunnen er situaties ontstaan waardoor deze gedwongen wordt tot snel intuberen. Het vergroot de veiligheid van de patiënt als de intubator beschikt over verschillende strategieën om een open luchtweg te kunnen waarborgen. De wijze waarop de intubatie wordt uitgevoerd, is afhankelijk van de anamnese, het lichamelijk onderzoek en de omstandigheden waaronder de intubatie moet plaatsvinden. Bij het zeer zieke kind worden de vitale functies geoptimaliseerd. Bij het niet-nuchtere kind past men de ‘rapid sequence induction’ toe waarbij het risico van aspiratie verminderd wordt. Reacties op manipulatie van de luchtwegen moeten worden gedempt met medicatie. Men kan kiezen uit een breed scala van kort- en (zeer) snelwerkende slaapmiddelen, pijnstillers en spierverslappers. Na intubatie beoordeelt men de positie van de tube met capnometrie, auscultatie en een röntgenfoto van de thorax. Het geven van de juiste sedatie en pijnstilling vergemakkelijkt de beademing na intubatie. Bij het kind met een moeilijke luchtweg is hulp van een anesthesioloog en een kno-arts onontbeerlijk.
