Activatie van plasma-eiwitten en bloedcellen bij het neonataal respiratoir distress-syndroom: pathogenetische en therapeutische aspecten

Samenvatting

Bij prematuren met het neonataal respiratoir distress-syndroom (rds) staan surfactanttekort en longonrijpheid centraal. Het is minder bekend dat bij prematuren met een ernstig rds gelijktijdige activatie van stolling, fibrinolyse, kinine-kallikreïne en complement in plasma optreedt. Dit activatieproces draagt bij aan het ziekteproces en gaat gepaard met activatie van circulerende neutrofiele granulocyten en trombocyten. Daarbij neemt het aantal circulerende neutrofiele granulocyten en trombocyten af. Perinatale asfyxie en longweefselbeschadiging door kunstmatige beademing initiëren en bevorderen dit activatieproces. Activatie van plasma-eiwitten en bloedcellen is positief gecorreleerd met de ernst van het neonataal rds, doordat het activatieproces bijdraagt aan het ontstaan van eiwitrijk oedeem in de kleinere luchtwegen. Naar verwachting zullen preventieve en therapeutische maatregelen gericht op activatie van plasma-eiwitten en bloedcellen, de mortaliteit en morbiditeit ten gevolge van het rds verder doen afnemen. Klinische trials naar effectiviteit en veiligheid van vroege dexamethasontoediening, antitrombinesubstitutie in combinatie met heparine, alsmede substitutie van α 1-proteïnase-inhibitor lijken in dit verband gerechtvaardigd.
