Abstract

Diagnostiek is vaak een onderbelicht deel van de geneeskunde. Het is echter een onmisbaar onderdeel. Zeker bij een aandoening als hartfalen is het belangrijk om voldoende zeker te zijn van de diagnose, alvorens men overgaat op behandeling en begeleiding. Daarnaast moet men als behandelaar niet alleen voldoende zeker zijn over de diagnose in geval van hartfalen, maar ook voldoende zeker over de oorzaak ervan. Dit betekent niet dat altijd alle vormen van diagnostiek hoeven te worden gebruikt om als behandelaar ‘100% zeker’ te zijn. Soms vallen om sociaal-medische (zeer hoge leeftijd, hoge immobiliteit), logistieke of financiële argumenten een aantal van de in dit hoofdstuk besproken diagnostische onderzoeken af. Zo is momenteel (Doppler)echocardiografie voor patiënten onder behandeling bij een huisarts of verpleeghuisarts niet zo gemakkelijk toegankelijk omdat een verwijzing naar de cardioloog hiervoor noodzakelijk is en er lange wachtlijsten bestaan voor echocardiografie door een tekort aan echolaboranten. Schoorvoetend komen er ‘open access’-echocardiografiefaciliteiten in Nederland, waardoor de toegankelijkheid voor echocardiografie verhoogd wordt.
