In my inaugural address I told you about my dream: Once upon a time there will be one physician for work and health, PWH in short. He will be in a multidisciplinary health care team in a hospital, have surgeries in a health centre for people with or without employment, and he will be financed from the basic health insurance. The PWH keeps in touch with employers, visits workplaces and, if necessary, intervenes in work systems in order to promote and maintain participation. The PWH thus forms the bridge between curative colleagues and the workplace. In social security the PWH has a monitoring role in the chain of recovery and participation. He proactively monitors stagnations in the chain of events towards re-establishing participation of disabled persons and vulnerable groups without an employer. When stagnation threatens, the PWH assesses functional abilities and, in cooperation with the disability employment advisor formulates a social-medical participation plan, in consultation with all concerned: the employer, the patient, the PWH colleague in healthcare and the curative colleagues. This also includes supporting participation in the form of (temporary) benefits or facilities, when necessary.
